By submitting this application, I am signing up my child for programs and/or camp operated by Chabad of Sea Cliff/Chabad of Brookville. I give permission for my child to participate in all activities, attend all trips on program-provided transportation, and receive medical treatment in the event of an emergency, G-d forbid.
I hereby release and hold harmless Chabad of Sea Cliff/Chabad of Brookville, its directors, staff, volunteers, and representatives from liability for accidents or injuries that may occur during activities related to Chabad of Sea Cliff/Chabad of Brookville programs, provided that normal safety procedures have been followed.
I grant permission to Chabad of Sea Cliff/Chabad of Brookville to photograph and videotape my child and to use such photos or videos (without identifying names) for promotional or educational purposes as deemed appropriate.
The parent/guardian signing this registration affirms that he/she has full legal authority to do so and agrees to be responsible for payment of all applicable tuition and fees.
I further acknowledge that certain activities may take place at off-site locations or third-party facilities, including recreational or swimming facilities. I agree to indemnify, release, and hold harmless Chabad of Sea Cliff/Chabad of Brookville and any applicable property owners or facility operators from any claims, costs, liabilities, damages, or demands arising from injury, illness, loss, or death related to my child’s participation in such activities, including the use of any facilities or equipment. This includes any damage to or loss of personal property.
I acknowledge that participation in these activities involves inherent risks and that I knowingly assume such risks on behalf of my child. By signing below, I confirm that I am of legal age, have read and fully understand this waiver, and agree to be legally bound by its terms
